About Rise

Institute

The Problem

The average inpatient rehab stay after a life-changing injury is just 37 days. After hospital discharge, individuals and their families face overwhelming obstacles and they attempt to navigate a new normal.

Unknowns

You’re on your own, trying to answer seemingly impossible questions: “Where do I go for therapy?” “How do I pay for it?” “Will I ever get better?”

Fragmented Care

You’re sent to different facilities across town for therapy, doctor appointments, wheelchair fittings, and mental health support. Nothing connects

Financial Barriers

Insurance runs out or denies your care. Bills pile up. Quality care becomes unaffordable.

Burden

Caregivers are exhausted and feel helpless. They would give the world to change things, but feel lost in the dark.

Rise exists to change this.
